Registration Process

Registering for participation in the 340B Program is quite simple. However, the details of the procedure could vary depending on the entity category your facility falls into. We take you through that process below.

DEADLINES
The quarterly deadlines for registration submission are:

  • December 1 for the quarter beginning January 1;
  • March 1 for the quarter beginning April 1;
  • June 1 for the quarter beginning July 1; and
  • September 1 for the quarter beginning October 1.

It is the entity's responsibility to tell its wholesaler or manufacturer that it is registered for 340B discount prices when it places an order.

The following form should be used by: HEALTH CARE CENTERS

  • Federal Qualified Health Center (FQHC)
  • FQHC-Look Alike
  • Family planning clinic
  • Ryan White/HIV clinic
  • Urban Indian Clinic/Tribal Clinic
  • Black Lung Clinics
  • Consolidated Health Centers
  • Migrant Health Centers
  • Health Care for the Homeless (Sec.330(h) PHSA)
  • Healthy Schools/Healthy Communities
  • Health Centers for Residents of Public Housing
  • Tribal Clinics or urban Indian organizations

Registration Forms

 

STD/TB CLINICS:
Clinics meeting the requirements for Sexually Transmitted Diseases and Tuberculosis entity eligibility must apply for the 340B Drug Pricing Program through their state program director. Annual certification is required for STD and TB clinics and takes place in the last calendar quarter each year.

Different states operate the application process differently: (1) some states certify the entire state or a local health department. That department then serves as the provider and supplies the individual clinics with the STD/TB medications or (2) the program director certifies individual entities and/or health departments. All certified entities (both by state and individual sites) are sent to the Centers for Disease Control and Prevention (CDC) for a second round of certification. The approved entities are listed in the 340B entity database.
